Online Enrollment

• Visit myapplications.microsoft.com -  If you are not on a computer connected to the COH network, you will be asked to log in with your City E-Mail address, network password, and a multi-factor authentication code.
• Select Paylogix App
• Click on Nationwide Pet Insurance near the bottom of the list of benefit options
• Click on the "Click to Learn More and Enroll" option on the left side of the page

Please note, you will now be routed to Nationwide’s web page for an insurance quote before enrollment is confirmed.
