Zenith American Solutions is the third party administrator for the Teamsters Local 14 Health Trust. For questions on claim status or coverage you must call Zenith directly. 

Participant login to the Zenith user portal will need to be set up with Zenith directly.  You may either call the number above or send an e-mail request at their website.  With login access you will be able to view you and your dependents benefit eligibility and claim status.

Teamsters Security Fund PPO Provider Network
Anthem BlueCross BlueShield. To find a PPO Provider, go to the Anthem website at www.anthem.com and:

  1. Under "Menu," click "Find a Doctor."
  2. Click "Continue" under "Search as a Guest."
  3. Under "How do you get insurance?" select "Through my employer."
  4. Select your State.
  5. Under "What type of care are you searching for?" select "Medical."
  6. Under "Select a plan/network," select "National PPO (Blue Card PPO)" and click "Continue."
  7. Provide the information requested and click "Search."

Health Plan of Nevada Point of Service Plan ( Ends 12/31/2025) - www.healthplanofnevada.com Click on the “HPN@YourService” link to set up your account to access HPN’s online member services. Click on New Users link and then select New Users again to register. Listed below are a few of the services you can access online:

  • Change your address
  • Request new ID cards
  • Verify your prescription drug coverage
  • Check your copayment amounts for medical services
  • Review the status of your claim
  • Find out who is on record as your primary care provider
  • Check the status of a prior authorization request
  • Find out how much has been applied toward your calendar year deductibl

New Plan Effective 01/01/2026- Engaged Care Plan 

The Engaged Care Plan promotes the Fund’s Family Wellness Centers, provided in partnership with Marathon Health, as your first stop and primary source for health care services.

In circumstances when you are unable to seek care at a Family Wellness Center (e.g., needing care after hours, in cases of emergency or medical urgency, or to seek out a specialist), the plan does provide access to the Anthem Blue Cross Blue Shield PPO national network. However, if you seek care outside the PPO network, services will not be covered (except in the case of an emergency or if referred by a Family Wellness Center).
